Baptist Health Lexington’s Anticoagulation Clinic provides education, point-of-care testing (Protime / INR), and dosing / administration instructions for patients on anticoagulant therapy, including warfarin (Coumadin®), dabigatran (Pradaxa®), rivaroxaban (Xarelto®), apixaban (Eliquis®), and edoxaban (Savaysa®). We also provide education and dosing recommendations for patients on enoxaparin (Lovenox®) and conduct a thorough medication reconciliation at each patient visit.
