Completing a birth plan can help ensure your labor and delivery experience reflects your personal vision and goals. The process is easy:

Use the form to list your preferences for labor and delivery, and discuss with your provider during a prenatal visit. When it’s time for baby’s arrival, present your birth plan to your labor and delivery nurse.
