Cancer Survivorship
Your cancer treatment is complete. So what comes next? Starting a new routine can be difficult following cancer treatment. The Cancer Survivorship Program at Baptist Health provides the expertise and support to help guide you and your loved ones through this transition.
The program encompasses the physical, psychosocial, functional
and spiritual needs of the survivor, the family and significant others, and
provides a road map for moving forward.
A Cancer Survivorship Visit includes:
- A one-time visit with the Cancer Survivorship Nurse Practitioner
- Review of your personalized treatment summary and survivorship care plan
- Physical examination
- Identification of current or potential side effects of cancer and cancer treatment
- Identification of strategies for management of side effects including making appropriate referrals
- Ordering of appropriate imaging or laboratory studies based on your symptoms
- Review of any imaging or lab results as appropriate
- Discussion of a personalized wellness and follow-up plan
The Cancer Survivorship Program
The Cancer Survivorship plan includes:
- Identification and management of the potential effects of cancer and its treatment
- Risk assessment and prevention resources for cancer spread, recurrence, and surveillance for secondary cancers
- Coordination of care, including guidelines and referrals for cancer screenings
Upon completion of your visit, the Nurse Practitioner will provide documentation to physicians involved in your care.
The Cancer Survivorship Program is for any Baptist Health cancer survivor who has completed treatment, including those who have been placed on oral maintenance or prophylactic therapy.
